Inquiries & New Clients
Potential new clients need to provide us with their name, date of birth, and phone number. Couples should provide their spouse’s information too. Address and email address are also preferred upon registration. Let me know if you would like appointment reminders by phone call, text, and/or email. If you plan to become a patient with Gianna of Albany, then you may also want to provide us with your health insurance information in advance.
