Insurance Prices

A health insurance claim for a 60-minute FertilityCare appointment is $200.00, but the actual amount that your health insurance company pays may vary. A co-pay will also be charged for your FertilityCare appointments if you normally have co-pays for your NaProTechnology appointments. For your convenience, the codes that we use are as follows:

Reproductive Care Management

Medical Couple:

If you are not paying with health insurance or if you are paying for items such as books, charts, stamps, or the no-show fee (which we do not bill to insurance), the prices are as follows:

Bill Pay

We submit health insurance claims and bill for co-pays and additional charges (such as for books, charts, and stamps) after your scheduled appointment time.

If you are a client or patient of Gianna of Albany, please pay your bill from your appointment with me directly to them in-person, in the mail, or in the Patient Portal.

Order Form (Ship Or Pick Up)

From me: I can send the Introductory Session Packet and User Picture Dictionary to new virtual clients (shipping charges apply). Supplies such as blank charts and stamps can be ordered from me for shipping or pick up to active clients only. We can arrange a pick-up day and time if you do not have a scheduled in-person appointment soon. Pick up from me in-person is preferred. Note that I can only ship materials from USPS on Saturdays and I will add on a shipping fee (to be paid directly to me), so please plan your orders accordingly.

Please contact me directly if you are interested in purchasing the Creighton Model FertilityCare System Charting App. The official FertilityCare App is available to established clients who have been charting on the paper chart for at least 5 follow-ups (3 months).

From the SPVI Institute: After the Introductory Session presentation as soon as the first follow-up is scheduled, I can order a new client’s materials (Introductory Session Packet and User Picture Dictionary) to be sent directly to the new client from the Saint Paul VI Institute Publications Department with the client’s written permission. The client will pay me for the cost of their materials and shipping charges. Please allow one week for shipping.

From Gianna of Albany: Alternatively, patients of Gianna of Albany can pick up supplies during their office hours at the Mechanicville location or call them directly to order and request shipping from their staff. In this case, billing would be done as usual through their system (not through me).
